The Capacity Works Referral Form helps us understand a participant’s strengths, goals, support needs, and current circumstances before services begin. Providing this information allows us to determine whether Capacity Works is the right fit and ensures support can be tailored to the participant from the outset.
The referral form can be downloaded, completed by the participant, family member, support coordinator, school, or allied health professional, and emailed to the Capacity Works administration team. Once received, we will review the referral and contact you to discuss the next steps.
